Friendship Club - Information for Referring Professionals

The Mental Health Association’s Friendship Club is a social club for people with mental illness who are in need of socialization, recovery based workshops and groups, daily living skills and the chance to be around accepting and understanding people. For each member of the Friendship Club to attend, they must meet certain criteria. They must be 18 or older, living in or receiving services from Cattaraugus County, be seeing a therapist and/or psychiatrist and have a primary diagnosis of mental illness. Each person applying for membership to the Friendship Club will have to have verification of an Axis I diagnosis of mental Illness.

The Therapist Verification Form (pdf) and Release of Information (pdf) are required to be filled out in full, before your client can attend the Friendship Club.
